慢性粒细胞白血病ZO-1基因启动子区甲基化状态分析及临床意义初步探讨

目的 通过对慢性期及急变期慢性粒细胞白血病(CML)患者骨髓标本ZO-1基因(人紧密连接蛋白1,ZO-1)启动子区甲基化状态的分析,初步探讨其在慢性粒细胞白血病病情发展变化中的临床意义。 方法 应用甲基化特异性聚合酶链反应(MS-PCR)检测10例慢性粒细胞白血病慢性期(CP)和10例慢性粒细胞白血病急变期(BP)患者的骨髓标本中ZO-1基因启动子区的甲基化状态。 结果 (1)在10例CML-CP患者中ZO-1基因启动子区呈非甲基化状态,而在7例CML-BP患者中ZO-1基因启动子区呈高甲基化状态,甲基化阳性率为70%(7/10)。(2)1例CML患者其慢性期骨髓标本ZO-1基因启动子区呈非甲基化状态,而急变期则呈甲基化状态,经治疗恢复到慢性期时,则又表现为非甲基化状态。结论 ZO-1基因启动子区甲基化状态可能与慢性粒细胞白血病病情发展变化密切相关,可能是一个提示慢粒急变的分子标志物。
Objective To investigate the methylation status of Zonula occludens 1(ZO-1) gene promoter in the chronic and blastic phase CML patients’ bone marrows,discuss its clinic sense in the pathogenesis and progression of chronic myelocytic leukemia. Methods The methylation pattern in promoter region of ZO-1 gene was detected with methylation specific PCR (MS-PCR) in bone marrow samples of 10 chronic phase CML patients and 10 blastic phase CML patientss .Results (1) The promoter region of ZO-1 gene was unmethylated in 10 cases of chronic phase patients;otherwise the methylation frequency of ZO-1 gene promoter region in 10 cases of CML-BP was 70% (7/10).(2) For one CML patient who progressed to BP and got back to CP after treatment, the promoter region of ZO-1 gene was unmethylated in chronic phase and methylated in blastic phase, otherwise, the methylation pattern of ZO-1 was trans back to unmethylated when the disease returned to the chronic phase. Conclusion The hypermethylated status of ZO-1 gene promoter region was specificitly detected in human CML blastic phase, it was closely correlated with the progression of the disease and might be a molecular maker which can prognosticate the progression to BP.
