???目的 检测脑卒中患者治疗前后脑区局部一致性的变化,试图阐明脑卒中患者肢体功能恢复的可能机制。方法 对18例缺血性脑卒中患者治疗前后及13例正常对照者进行静息态功能磁共振扫描,计算大脑局部一致性(ReHo),同时对患者治疗前后进行肢体运动功能评定(FMA评分),使用统计学分析方法对结果进行组间比较及统计分析。结果 患病组治疗后FMA评分明显高于治疗前。患病组治疗前与对照组相比ReHo值升高的脑区位于左侧颞中回;ReHo值降低的脑区为两侧顶上小叶、枕叶中部,右侧枕叶下部(BA18),左侧枕叶、左侧楔前叶、眶回(BA11)等。患病组治疗后与对照组对比ReHo值升高的脑区为左侧枕叶舌回、颞叶梭状回、颞中回和脑桥等;ReHo值降低的脑区为两侧小脑、两侧颞下回等。结论 利用静息态功能磁共振可以对缺血性脑卒中患者的功能恢复情况进行早期评估,为研究脑卒中患者功能恢复机制及评价功能康复提供依据,并更好地指导脑卒中的治疗。??

Objective We detect the changes of brain regional homogeneity of the ischemic stroke patients before and after treatment,illustrating the possible mechanisms of stroke patients with limb functional recovery。Methods We do resting-state fMRI scan for 18 cases of ischemic stroke patients before and after treatment and 13 cases of normal control group ,and calculate regional homogeneity value of all local brain regions. We also assess the limb motor function of the patients before and after treatment(FMA score), and use the statistical analysis method for the results compared between groups and statistical analysis.Results The FMA score of ischemic stroke patients before treatment are significantly higher than that before treatment.Comparing with the control group, cerebral region of increasing ReHo value in ischemic stroke patients before treatment is left middle temporal gyrus; cerebral regions of decreasing ReHo value are bilateral superior parietal lobule, medial occipital lobe, right inferior occipital lobe, left occipital lobe, left precuneus, orbital gyrus, etc.Comparing with the control group, cerebral regions of increasing ReHo value in ischemic stroke patients after treatment are lingual gyrus of left occipital lobe, fusiform gyrus of left temporal, left middle temporal lobe, pons, etc,Cerebral regions of decreasing ReHo value are located in the bilateral cerebellum,bilateral inferior temporal gyrus, etc.Conclusion Resting-state fMRI can be used to assess the functional recovery of ischemic stroke patients, further study the mechanism of functional recovery in stroke patients , and better guide the treatment of stroke.?????
