目的 将体外培养的胚胎后肾间充质细胞移植到大鼠右肾包膜下,探讨移植细胞对宿主急性肾小管坏死的保护作用。方法 RIMM-18细胞体外培养及扩增用于移植,SD大鼠分为五组,除正常对照组外其他四组均庆大霉素200mg/(kg.d)连续4天皮下注射诱导建立肾毒性肾小管坏死模型。首剂庆大霉素注射后第5、8、11、14天每组分别取6只大鼠处死,留取血和肾组织标本,检测以下指标:①DAPI示踪;②Scr和尿NAG酶;③肾组织病理评分;④肾小管上皮细胞Ki-67的表达,计算增殖指数。⑤肾小管上皮细胞的凋亡,计算凋亡指数;⑥肾小管凋亡相关因子Bcl-2蛋白表达。结果 (1)成功体外培养及扩增RIMM-18细胞系。(2)细胞移植组大鼠: ①死亡率明显降低;②肾功能指标改善;③右肾皮质病理积分降低;④右肾肾小管上皮细胞增殖明显;⑤右肾肾小管上皮细胞凋亡减少;⑥右肾Bcl-2蛋白表达水平增强。 结论 肾包膜下移植胚胎后肾间充质细胞能够修复损伤的肾脏,改善肾功能。

Objective The subcapsular transplantation of metanephric mesenchymal cells (MMCs) may be a new therapeutic approach for the treatment of acute tubular necrosis (ATN). To investigate this hypothesis, we evaluated the nephroprotective effects of transplanting MMCs into the renal subcaspsule of rats with ATN induced by gentamicin. Methods MMCs were expanded in culture and immunocytochemistry was used to characterize the cells. After gentamicin-induced ATN, fluorescently-labeled cells were transplanted and traced in kidney tissues by fluorescence microscopy. Renal function was tested by serum creatinine(Cr), N-acetyl-b-D-glucosaminidase(NAG); kidney pathology was studied by hematoxylin-eosin staining; apoptosis was examined by the TUNEL assay, and Ki-67 and Bcl-2 expression were examined by immunohistochemistry respectively. Results (1) MMCs were expanded in culture and the phenotype of the cells was vimentin-positive and keratin-negative. (2) In the MMCs-treated group: the mortality rate decreased; Cr and NAG clearly decreased; In the MMCs-treated kidneys: the histopathologic lesion scores were lower; the Ki-67 antibody and apoptosis of renal tubular epithelial cells was improved or reduced respectively; the expression of Bcl-2 protein was upregulated. Conclusions The subcapsular transplantation of MMCs could ameliorate renal function and repair kidney injury.
